How to Get a Pathology Request

From Your GP
Your GP gives you a pathology request form, paper or electronic, at your appointment based on what needs testing.

At the Collection Centre
Take your form to Australian Clinical Labs. Some tests require fasting — your GP will let you know beforehand.
