GPMP and TCA — What Is the Difference?

GP Management Plan (GPMP)
A GPMP is a personalised plan your GP creates with you. It documents your chronic conditions, sets health goals, and outlines the steps your care team will take. It is the foundation of structured chronic disease care at our practice.

Team Care Arrangement (TCA)
A TCA coordinates care between your GP and at least two other healthcare providers. It unlocks up to 5 Medicare-subsidised allied health visits per calendar year — including physio, podiatry, dietitian, exercise physiologist, and more.
Who Can You See Under a Care Plan?

Physiotherapist
For chronic pain, mobility issues, post-injury recovery, and musculoskeletal conditions that affect daily life.

Dietitian / Nutritionist
For diabetes management, weight, cholesterol, gut health, and chronic disease nutrition support.

Psychologist
For mental health conditions linked to chronic illness — anxiety, depression, and chronic pain adjustment

What is Care Plans (GPMP & TCA)?
A care plan is a longer appointment for patients who have a long-term medical condition and need regular, coordinated care. Your GP works with you to document your health conditions, set goals, and arrange the right support team. There are two types: a GP Management Plan (GPMP), which is your personalised health plan, and a Team Care Arrangement (TCA), which coordinates that plan with at least two other healthcare providers and unlocks Medicare-subsidised allied health visits.

What We Cover
Nurse assessment — vitals, medications, and recent test results reviewed before your GP appointment
GP consultation — your conditions, goals, and care plan are discussed and formalised
Allied health referrals — up to 5 subsidised visits per year under a TCA
Written care plan — you receive a copy to keep
Annual review — your plan is reviewed at least once per year and updated as needed
Coordination between your GP and allied health providers
